DOCUMENTATION REQUIRED Valid Proof of identification - Picture ID Social Security Cards for you, your spouse and dependents Birth dates for you, your spouse and dependents on the tax return Wage and earning statement(s): Form W-2, 1099 Interest and dividend statements from banks (Forms 1099) 1095 for health insurance - List of everyone in your household who had health insurance 1098 for education credit plus detailed financial statement from the school Bank Routing Numbers and Account Numbers for Direct Deposit Total paid for day care provider and the day care provider's ID information Summarized list of the other income/expenses for thsoe who itemize or are self employed A copy of last year's Federal and State returns, if available Tax identifying number (the provider's Social Security Number or the provider's business Employer Identification Number) Any additional information tax information that may be required - call for additional information To file taxes electronically on a married filing joint tax return, both spouses must be present to sign the required forms.

Eligibility

Targets low to moderate income families (less than $52,000 in adjusted gross income) and families who qualify for earned income credit.
