Must provide daycare provider's tax identifying number such as their Social Security Number or business Employer Identification Number (EIN). * Summarized list of other income/expenses for those who itemize or are self-employed Note: If taxpayer purchased health insurance through the Marketplace, established by the Affordable Care Act, then taxpayer must bring one of the following forms: Forms 1095-A, B or C, Affordable Health Care Statements.
